This Privacy Policy details the policies used by  us in collecting information from  ("website" or "site"), and how the information is stored, used, shared, and protected.

Application

In this Privacy Policy:

personal information” has the definition given to it under the Privacy Act.

“privacy act” means the Privacy Act 1988 (Cth).

”sensitive information” has the definition given to it under the Privacy Act.

”stakeholders”, “you”, “your” and “yourself” refers to the individual about whom we collect personal information.

Commencement

The Privacy Act places restrictions on us collecting sensitive information about you (which includes information about your religion, political views, ethnicity, criminal records, health information and sexual preferences). There may be times where we need to collect sensitive information, for instance if we are providing services to you and you wish to disclose any health information to us to allow us to tailor the services to your needs or if you are applying for a job with us. However, we would not do so without first obtaining your consent.

We understand that when dealing with us, some members of the public may wish to remain anonymous or use a pseudonym. On certain occasions, one of our employees may need to identify the person requesting the information. This is usually done by asking the stakeholder to confirm their identity by providing a copy of the stakeholder’s license or coming into our office in person. By taking this extra step, we are ensuring that the personal information provided is going to the correct person and that personal information is not accessed incorrectly or fraudulently.

Lodging a Complaint and Complaint Handling

The privacy officer will first consider your complaint to determine whether there are simple or immediate steps which can be taken to resolve the complaint. We will generally respond to your complaint within a week.

If your complaint requires more detailed consideration or investigation, we will acknowledge receipt of your complaint within a week and endeavour to complete our investigation into your complaint promptly. We may ask you to provide further information about your complaint and the outcome you are seeking. We will then typically gather relevant facts, locate and review relevant documents and speak with individuals involved.

In most cases, we will investigate and respond to a complaint within 30 days of receipt of the complaint. If the matter is more complex or our investigation may take longer, we will let you know.

If you are not satisfied with our response to your complaint, or you consider that Club Upgrade may have breached the Australian Privacy Principles or the Privacy Act, a complaint may be made to the Office of the Australian Information Commissioner.
